Cosmelan & Dermamelan Depigmentation Treatment
Cosmelan and Dermamelan are professional depigmentation peels, specifically formulated to treat various types of skin pigmentation including melasma, sunspots, age spots, and other hormonal pigmentation. Cosmelan and Dermamelan work by inhibiting tyrosinase, the enzyme responsible for production of melanin in the skin through corrective and preventative actions:
𝐂𝐎𝐑𝐑𝐄𝐂𝐓𝐈𝐕𝐄: Inhibits melanin synthesis, stimulates cell renewal, and reduces the accumulation of melanin in the superficial skin layer. This fades away existing pigmentation, resulting in a brighter and more even skin tone.
𝐏𝐑𝐄𝐕𝐄𝐍𝐓𝐀𝐓𝐈𝐕𝐄: By inhibiting tyrosinase activation and thereby reducing its function, the melanin is prevented from reaching the skin surface. This decreases the formation of new unwanted pigmentation.
Dermamelan works similarly to Cosmelan, but is approximately 30% stronger in its corrective and regulating action, containing higher amounts of active ingredients. Due to its potency, it requires a prescription and is performed by medical professionals only.
Both Cosmelan and Dermamelan are non-invasive, and have a high safety margin. No aggressive skin-bleaching agents or hydroquinone are used in the formulation of these treatments, making it suitable for virtually all skin types (Fitzpatrick I-VI).
Used to Treat
- Melasma: Uneven and irregular with undefined edges. Shadow-like appearance. Commonly triggered by hormonal changes.
- Post inflammatory Hyperpigmentation (PIH): Irregular, flat, asymmetric spots of dark pigmentation, either localized or dispersed. Commonly resulting from inflammation or trauma (e.g. acne).
- Solar Lentigo: Spots that are oval or round, lentil-shaped and even, measuring 2-20 millimetres. Caused by UV damage.
- Senile Lentigo: Spots that are oval or round and even, measuring 2-20 millimetres. Cumulative effect of sun damage.
- Ephelides (Freckles): Multiple small, flat, pigmented spots. Light brown in colour and symmetrical.
Treatment Information
Treatment Preparation
- Avoid direct and intense sun exposure (e.g. tanning and solariums) for at least 4 weeks prior to treatment.
- Avoid waxing, threading, or other forms of hair removal for at least 1 week prior.
- Avoid retinol, alpha-hydroxy acids (AHAs), and other exfoliating products for at least 1 week prior.
- Smoking should be avoided for at least 1 week prior as it can reduce the skin’s ability to heal and increase the risk of complications.
- Avoid chemical or physical exfoliation for at least 1 week prior.
- If you have an active infection, such as a cold sore or skin rash, please notify as soon as possible. Treatment must be delayed until the infection has resolved to avoid complications.
- Please inform us of any known history of hypersensitivity or allergic reactions.
What to Expect
Prior to treatment, our Nurse and Doctor will conduct a comprehensive consultation to assess your medical history and suitability to treatment. They will also discuss your treatment goals, address any concerns, and answer any questions you may have.
Both Cosmelan and Dermamelan consist of in-clinic treatments and a home-care regimen:
Step 1: Mandatory Skin Preparation
- The Mesoestetic Brightening Peel Booster will be used once every evening for 4 weeks leading up to your treatment to enhance results and decrease peeling post treatment.
- A Mesoestetic 30% Lactic Peel is performed at 2 weeks prior to ensure skin is optimally prepped, healthy, and hydrated for your treatment.
Step 2: In-Clinic Application
- The clinician will thoroughly cleanse and degrease the skin for optimal application.
- The Cosmelan/Dermamelan 1 mask is applied evenly to the entire face and left on for 8-12 hours. For this reason, your treatment will be scheduled first thing in the morning.
- The clinician will provide you instructions for mask removal and a home care kit for continued depigmentation and maintenance.
Step 3: Treatment Follow-Up
- Patient will return to clinic on day 3 and day 7-10 for post treatment for follow-up.
- At each follow-up appointment:
o A 20 minute LED Phototherapy Session is performed to optimise your healing response by stimulating macrophage and cytokine activity.
o A Post-Peel Crystal Fibre Mask is applied to calm and soothe the skin after treatment and promote healing and recovery.
Possible Side Effects
As both Cosmelan and Dermamelan are intensive clinical treatments, approximately 5 days of planned downtime is recommended.
Day of Application: Very mild tingling sensation or itchiness is sometimes felt towards the end of treatment and right before removal. After removal, skin will feel mildly hot and tight.
Day 1 Post-Treatment: Skin will be red/pink in appearance, sensitivity is increased and feel hot to the touch. Swelling and inflammation is visible.
Days 2-3: Skin will feel dry and tight, particularly around the mouth, chin and nose. Dry patches of skin begin to darken and peeling will usually commence. Sun exposure must be avoided and regular application of SPF 50+ is mandatory to protect your skin and results.
Days 4-6: Skin may feel sensitized and irritated. Inflammation, redness and peeling begin to gradually decrease.
Days 7-10: Peeling and redness decreased and continues to settle. Residual dryness may be present.
Improvement in skin texture and reduction in pigmentation may visible as soon as 7-14 days following treatment, with continued improvement seen over the next 1 month.
Cold Sores: Prophylactic antiviral medication may be taken prior to, and after treatment to minimize risk of eruption.
Skin Purging: Chemical peels speed up cell turnover, which may cause acne-causing bacteria and other impurities to come up to the surface of the skin more quickly, causing a temporary flare-up of acne. Purging is usually temporary and will clear up as the impurities are eliminated.
Treatment Aftercare
- Use a broad-spectrum sunscreen with an SPF 50+ and reapply as needed to protect the skin at all times.
- Do not touch, pick or peel at any flaking or peeling skin as this can affect the results and increase the risk of complications such as scarring and post inflammatory hypo/hyperpigmentation.
- Do not apply makeup or other cosmetic products for 48 hours post-treatment.
- Avoid sun exposure and exposure to heat sources (e.g. hot showers, spas, saunas, swimming pools, strenuous exercise and sweating) for the first 72 hours.
- Swelling can be managed with intermittent cool compress application and anti-histamines.
- Drink plenty of water to help your skin recover and stay moisturized.
Immediate Aftercare
- Your clinician will instruct you of the appropriate removal time for your Cosmelan/Dermamelan treatment.
- On completion of application time, remove the mask with lukewarm water. You face will be warm and red.
- Apply Melan Recovery.

- Dermamelan Treatment Cream on its own (or if not tolerated mixed with equal parts Melan Recovery Cream and Hydra-Vital Factor K, applied over the top).
- Hydra Vital Factor K (if additional soothing support required use Melan Recovery) .
- SPF – Mesoprotech Melan 130 Pigment Control.
MIDDAY
- Wipe over skin with hydratonic and makeup pad.
- Dermamelan Treatment Cream mixed with Melan Recovery Cream over areas of concern e.g. melasma spots, darker pigmentation.
- SPF – Mesoprotech Melan 130 Pigment Control.
PM
- Hydracream Fusion followed by Hydratonic Mist.
- EVERY ALTERNATIVE NIGHT – Hydra-vital Face Mask for 20 minutes.
- Dermamelan Treatment Cream on its own (or if not tolerated mixed with equal parts Melan Recovery Cream and Hydra-Vital Factor K, applied over the top).
Hydra Vital Factor K (if additional soothing support required use Melan Recovery).
4 Weeks Onwards (Continue for Following 5-6 months Post Treatment)
AM
- Hydracream Fusion followed Hydratonic Mist.
- Anti-ageing/treatment active of choice – Pigment suppressor, Vitamin C and antioxidants recommended (please see your dermal therapist for the recommended product).
- SPF – Mesoprotech Melan 130 Pigment Control.
PM
- Hydracream Fusion followed by Hydratonic Mist.
- Dermamelan Treatment Cream on its own, or if not tolerated mixed with equal parts Melan Recovery Cream and Hydra-Vital Factor K, applied over the top.
- Anti-ageing/treatment active of choice eg Vitamin A/Retinol – (please see your dermal therapist for the recommended product).
Hydra Vital Factor K (if additional support required use Melan Recovery).
Follow Up & Maintenance
Both Cosmelan and Dermamelan are intensive treatments requiring a minimum 4-6 month commitment to meticulous home care regimen. Strict adherence is essential for removing accumulated melanin from the outer layers of the skin, as well as effectively regulating its overproduction to maintain long-term results. Daily use of SPF 50+, avoidance to sun exposure and other triggers must also be ongoing.
For most patients, only 1 treatment is required. In cases of severe pigmentation, treatment may need to be repeated maintain results and prevent recurrence. Additional treatment may be required if aftercare instructions or homecare regimen was not followed correctly.
Treatment may be repeated once every 6 months to 1 year.
Who is Not Suitable
- Recent sunburn or significant sun exposure within the last month.
- Pregnant or breastfeeding.
- Known hypersensitivity or allergy to any chemical peel solution and/or ingredients.
- Taking oral Isotretinoin or other retinoids within the last 6 months.
- Autoimmune diseases affecting the skin, chronic imbalances and/or dermatological disease.
- Active skin cancers or lesions.
- Recent chemotherapy or radiotherapy.
- Recent facial or dermatological surgery.
- Recent cryosurgery or liquid nitrogen treatment.
- Known history of keloids or hypertrophic scarring.
- Active skin infection or acne, open wounds, recurring facial warts, or cold sores.
